Consent and Confirmation of Details *

I consent to my child attending NewHope Youth (NewHope Baptist Church's Youth Ministry). I will encourage my child to cooperate with the leaders and other children. I agree to pick up my child promptly at the advertised conclusion time of the youth program (9:30pm unless otherwise stated). I acknowledge that I am responsible for the collection of my child from the program. I authorise the leader in charge to arrange for my child to receive first aid or medical treatment as deemed necessary (parents/guardians will always be contacted in the event of any incident or emergency). I authorise an appointed representative of NewHope Youth to take and use for the reasonable purposes of NewHope Baptist Church any picture (including photographic, motion picture, and electronic (video) images) and voice (including sound and video recordings) or other recordings of my child, inline with NewHope's privacy policy: newhope.net.au/privacy. This material may be published in any of NewHope’s media channels, including web, print and broadcast media.
